Senator Grassley provides services for Iowans who are having problems with a federal agency. To request assistance, please fill out the following information. This will be used to generate the Privacy Release Form necessary to allow Senator Grassley’s team of dedicated constituent service specialists to assist you. A signed copy of this form is required to begin the process. If you need assistance completing your form, please contact any one of Senator Grassley’s Iowa-based offices.

    Be sure to fill in all the required information as best as possible.

  2. Print & Sign

    Be sure to sign the completed Privacy Release form. Federal Law prohibits Senator Grassley's staff from accessing your confidential information without your signature.

  3. Return Signed Form

    Mail your signed Privacy Release Form and any associated material to Senator Grassley's nearest Iowa-based office or submit it via email.
